Zbog valova velikih vru\u0107ina o\u010dekuje se porast prevalencije mnogih zdravstvenih problema. <\/p><p>Visoke temperature, vru\u0107e i sparno vrijeme, s visokim postotkom vlage u zraku, predstavljaju optere\u0107enje za organizam. <\/p><p><strong>Tko je pod pove\u0107anim rizikom?<\/strong><\/p><p>Opasnosti pregrijavanja tijela osobito su izra\u017eene kod kroni\u010dnih bolesnika i starijih osoba, \u010diji je organizam ve\u0107 oslabljen brojnim bolestima. Naime, fizi\u010dke promjene metabolizma koje nastaju s godinama nepovoljno utje\u010du na mehanizam regulacije tjelesne temperature. Ko\u017ea starijih ljudi je tanja i nudi slabiju za\u0161titu od sunca. Slabljenje kognitivnih sposobnosti tako\u0111er mo\u017ee utjecati na senzornu sposobnost starijih osoba i sprije\u010diti ih da &#8220;osje\u0107aju&#8221; toplinu. Osobe s demencijom mo\u017eda ne\u0107e osjetiti toplinu i \u010desto \u0107e odjenuti previ\u0161e slojeva odje\u0107e.<\/p><p>Zdravstveni problemi koji pove\u0107avaju rizik od hipertermije:<br \/>\u00a0 \u2022 oslabljen rad \u017elijezda znojnica je dio fiziolo\u0161kog procesa starenja<br \/>\u00a0 \u2022 kardiovaskularne i cerebrovaskularne bolesti, bolesti plu\u0107a i bubrega, \u0161e\u0107erna bolest <br \/>\u00a0 \u2022 oboljeli od Alzheimerove bolesti i drugih demencija<br \/>\u00a0 \u2022 osobe na terapiji diureticima, sedativima, anksioliticima, lijekovima za sr\u010dane bolesti i povi\u0161eni krvni tlak <br \/>\u00a0 \u2022 debljina<br \/>\u00a0 \u2022 pijenje alkoholnih pi\u0107a i velikih koli\u010dina kave (koja djeluje dehidriraju\u0107e)<\/p><p><strong>Posljedice po zdravlje<\/strong> <\/p><p>Toplinski poreme\u0107aji (toplinska sinkopa, toplinski gr\u010devi, sun\u010danica, toplinska iscrpljenost i toplinski udar) skupina su srodnih bolesti uzrokovanih dugotrajnom izlo\u017eeno\u0161\u0107u visokim temperaturama, ograni\u010denim unosom teku\u0107ine ili poreme\u0107ajem tjelesnih mehanizama za reguliranjem tjelesne temperature. Pod normalnim okolnostima tijelo se hladi znojenjem. Me\u0111utim u uvjetima jako visoke temperature, a naro\u010dito pove\u0107ane vla\u017enosti zraka, sam mehanizam znojenja nije dovoljan. Znoj ne\u0107e isparavati s ko\u017ee, \u0161to onemogu\u0107ava otpu\u0161tanje tjelesne topline, a tjelesna temperatura raste.<br \/>\u00a0 \u2022 <strong>Toplinska sinkopa<\/strong> je stanje iznenadne vrtoglavice i naglog gubitka svijesti koje nastaje usljed tjelesne aktivnosti u uvjetima povi\u0161ene vanjske temperature. Potrebno je skloniti osobu s vru\u0107ine u hladniji prostor, staviti noge u povi\u0161eni polo\u017eaj i kada osoba do\u0111e svijesti dati joj piti vodu ili prirodne negazirane vo\u0107ne sokove.<br \/>\u00a0 \u2022 <strong>Toplinski gr\u010devi<\/strong> su bolni gr\u010devi mi\u0161i\u0107a koji naj\u010de\u0161\u0107e zahva\u0107aju ruke, noge ili mi\u0161i\u0107e trbuha. Javljaju se usljed napornog fizi\u010dkog rada na visokim temperaturama, kada se osoba jako znoji, a nedovoljno nadokna\u0111uje izgubljenu teku\u0107inu i elektrolite. Iako tjelesna temperatura i puls obi\u010dno ostaju normalni tijekom toplinskog gr\u010da, ko\u017ea je \u010desto vla\u017ena i hladna. Odmah treba prestati s fizi\u010dkom aktivno\u0161\u0107u i skloniti se u sjenu ili klimatizirani prostor. Nadoknaditi izgubljenu teku\u0107inu i sol (piti vodu sa soli, prirodne negazirane vo\u0107ne sokove, nikako alkohol i kofeinske napitke!).<br \/>\u00a0 \u2022 <strong>Toplinska iscrpljenost<\/strong> je porema\u0107aj organizma koji ukazuje na smanjenu sposobnost otpu\u0161tanja topline iz tijela. Simptomi koji ukazuju na toplinsku iscrpljenost su umor, slabost, glavobolja, mu\u010dnina, hladna i vla\u017ena ko\u017ea, ote\u017eano znojenje, ubrzani puls. Odmah se treba skloniti u hladniji prostor i piti puno teku\u0107ine. Ukoliko se osoba ubrzo ne osje\u0107a bolje potrebno je pozvati lije\u010dni\u010dku pomo\u0107.<br \/>\u00a0 \u2022 <strong>Toplinski udar<\/strong> je najte\u017ei oblik hipertermije. Sli\u010dan je sun\u010danici, ali on ne mora nastati kao direktna posljedica izlaganja suncu. Naime, toplinski udar nastaje zbog povi\u0161ene tjelesne temperature i nemogu\u0107nosti organizma da temperaturu odr\u017ei u normalnim granicama. Termoregulacijski mehanizmi organizma prestaju funkcionirati, a tijelo nije vi\u0161e u stanju otpu\u0161tati vi\u0161ak topline u okolinu. Naj\u010de\u0161\u0107e se javlja za vrijeme ljetnih sparina kada je vla\u017enost zraka visoka, pa su znojenje i prirodno hla\u0111enje organizma ote\u017eani. Posljedi\u010dno raste i unutarnja temperatura tijela.<\/p><p>Znaci toplinskog udara:<br \/>&#8211; nastaje naglo, osoba je zbunjena i dezorijentirana s iznenadnim gubitkom svijesti i padom tlaka<br \/>&#8211; ko\u017ea je jako vru\u0107a (osjeti se na dodir rukom), suha je i crvena<br \/>&#8211; puls je jedva pipljiv i ubrzan (do 160-180 u min.)<br \/>&#8211; tjelesna temperatura je povi\u0161ena, preko 39\u00b0C<\/p><p>Toplinski udar u najte\u017eim slu\u010dajevima zavr\u0161ava komom i smr\u0107u. Temperatura vi\u0161a od 40\u00b0C trajno o\u0161te\u0107uje unutarnje organe i mozak. Odmah treba pozvati lije\u010dni\u010dku pomo\u0107, a osobu prenijeti u hladniji prostor, smjestiti je u vodoravan polo\u017eaj, skinuti joj odje\u0107u i po\u010deti s vanjskim hla\u0111enjem \u2013 hladni oblozi na vrat, pazuhe i prepone, \u0161pricati ko\u017eu vodom temperature 25-30\u02daC (ne ledenom vodom!) uz istovremeno stvaranje umjetnog vjetra ventilatorom, lepezom ili novinama. Ne stavljati led na ko\u017eu (opasnost od vazokonstrikcije krvnih \u017eila ko\u017ee i stvaranja daljnje nemogu\u0107nosti otpu\u0161tanja topline iz tijela). Hla\u0111enje treba trajati sve dok se tjelesna temperatura ne spusti ispod 38\u00b0C. Ne davati acetilsalicilnu kiselinu ili paracetamol za sni\u017eavanje temperature! Osobe bez svijesti postaviti u bo\u010dni le\u017ee\u0107i polo\u017eaj.<\/p><p>Po\u0161to su toplinom uzrokovani poreme\u0107aji preventabilni, va\u017eno je znati koje su najvulnerabilnije skupine i \u0161to sve treba poduzeti da ne do\u0111e do razvoja poreme\u0107aja izazvanih visokim temperaturama okoline.<\/p><p>\u00a0<\/p><p><strong>Preporuke za za\u0161titu od vru\u0107ina za osobe starije \u017eivotne dobi<\/strong><\/p><p>\u00a0 \u2022 Planirati dnevne aktivnosti s obzirom na vru\u0107inu (fizi\u010dki zahtjevnije poslove obaviti ujutro i nave\u010der). <br \/>\u00a0 \u2022 Starije osobe ne smiju se izlagati suncu u razdoblju od 10-17 sati, posebno ne stariji kroni\u010dni bolesnici oboljeli od kardiovaskularnih, cerebrovaskularnih, respiratornih bolesti, renalne insuficijencije, dijabetesa.<br \/>\u00a0 \u2022 Rashladiti svoj dom (zasjeniti prozore, no\u0107u i rano ujutro otvoriti prozore), boraviti \u0161to vi\u0161e u klimatiziranim prostorima.<br \/>\u00a0 \u2022 Prilikom izlaska iz ku\u0107e starija osoba treba nositi udobnu laganu odje\u0107u, svijetlih boja od prirodnih materijala i za\u0161tititi glavu \u0161e\u0161irom, maramom ili kapom.<br \/>\u00a0 \u2022 Ne \u010dekati da se razvije osje\u0107aj \u017ee\u0111i, nego piti tijekom dana \u010desto i po malo! Nu\u017eno je dnevno uzimanje do 2 litre teku\u0107ine (negazirana voda, juhe, variva ili \u010dajevi), a prednost dati laganoj hrani (jesti sezonsko vo\u0107e i povr\u0107e). <br \/>\u00a0 \u2022 Osim vode va\u017eno je nadoknaditi i znojenjem izgubljene soli da ne bi nastupilo stanje disbalansa elektrolita u organizmu (opasnost od toplinskih gr\u010deva!).<br \/>\u00a0 \u2022 Svi koji iz zdravstvenih razloga moraju ograni\u010diti unos teku\u0107ine i soli svakako se trebaju posavjetovati s lije\u010dnikom. Osim pridr\u017eavanja uputa o lije\u010denju i kontrolnih uputa lije\u010dnika o uzimanju lijekova nu\u017ena je u\u010destalija kontrola krvnog tlaka u starijih osoba.<br \/>\u00a0 \u2022 Izbjegavati alkohol, kavu, vru\u0107u i \u201cte\u0161ku\u201d hranu (pr\u017eena, pohana, slatka, jako zasoljena i za\u010dinjena jela) jer jo\u0161 dodatno zagrijavaju organizam. <br \/>\u00a0 \u2022 Izbjegavati pijenje prehladne teku\u0107ine (\u017eelu\u010dani gr\u010devi!).<br \/>\u00a0 \u2022 Va\u017eno je redovito odr\u017eavati osobnu higijenu, tu\u0161irati se ili kupati u mlakoj vodi.<br \/>\u00a0 \u2022 Pojava glavobolje, mu\u010dnine, povra\u0107anja, vrtoglavice, gr\u010deva u mi\u0161i\u0107ima, hladne i vla\u017ene ko\u017ee, ubrzanog pulsa znak su uzbune za stariju osobu, osobito bolesnu i funkcionalno onesposobljenu. Starija osoba mora se hitno javiti svom izabranom doktoru obiteljske medicine ili najbli\u017eoj hitnoj pomo\u0107i. <br \/>\u00a0 \u2022 Starije osobe zbog smanjenih aklimatizacijskih mogu\u0107nosti, fiziolo\u0161ki nisu u stanju u\u010dinkovito se rashladiti kad se temperatura podigne iznad 26\u00b0C. Zato svaka ustanova koja skrbi za starije osobe treba osigurati sobu ili prostor koji odr\u017eava temperaturu na 26\u00b0C ili ni\u017ee.<br \/>\u00a0 \u2022 Za starije osobe je va\u017ena i stalna tjelesna i psihi\u010dka aktivnost. Osobito je korisno kretati se u jutarnjim i ve\u010dernjim satima, po mogu\u0107nosti u prirodi (parkovi, \u0161etnice uz more, boravak u vrtovima i sl.). Ukoliko je kretanje ote\u017eano, tada je nu\u017eno redovito vi\u0161esatno provjetravanje prostorija u kojoj starija osoba boravi.<br \/>\u00a0 \u2022 Prilikom izlaska iz klimatizirane prostorije, ukoliko je velika temperaturna razlika u prostoriji i vanjske temperature (ve\u0107a od 7\u00b0C), izlaz mora biti postepen jer se stariji organizam u kratkom vremenu ne mo\u017ee adaptirati na naglu promjenu temperature.<br \/>\u00a0 \u2022 Strogo se treba pridr\u017eavati uputa lije\u010denja i kontroliranog uzimanja lijekova te dr\u017eati lijekove na temperaturi ispod 25\u00b0C ili u hladnjaku (pro\u010ditati upute o \u010duvanju lijekova u uputi o lijeku). Tra\u017eite savjet lije\u010dnika ako imate neku kroni\u010dnu bolest ili uzimate vi\u0161e lijekova, pogotovo lijekove za poja\u010dano mokrenje i sni\u017eavanje tlaka.<br \/>\u00a0 \u2022 Starija osoba obavezno uz sebe uvijek mora imati svoje osnovne podatke: ime i prezime, godina ro\u0111enja, adresa stanovanja i broj telefona kontakt osobe kojoj se mo\u017ee javiti u slu\u010daju potrebe. <br \/>\u00a0 \u2022 \u010clanovi obitelji, prijatelji, susjedi, poznanici, \u010dlanovi udruga koje skrbe za starije nu\u017eno moraju u vrijeme vru\u0107ih ljetnih mjeseci u\u010destalije kontaktirati stariju osobu, a u slu\u010dajevima njezine slabije funkcionalne sposobnosti \u010dak i svakodnevno po nekoliko puta.<\/p><p>\u00a0<\/p><p><strong>Va\u017eni brojevi telefona<\/strong><br \/><strong>Hitna pomo\u0107 \u2013 194<\/strong><br \/><strong>Centar za hitne situacije \u2013 112<\/strong><br \/><strong>Inga Vu\u010dica, dr. med., univ. mag. sanit. publ., spec. javnog zdravstva<\/strong><br \/><strong>Slu\u017eba za javno zdravstvo i promicanje zdravlja NZJZ SD\u017d<\/strong><\/p>\t\t\t\t\t\t\t\t<\/div>\n\t\t\t\t<\/div>\n\t\t\t\t\t<\/div>\n\t\t<\/div>\n\t\t\t\t\t<\/div>\n\t\t<\/section>\n\t\t\t\t<\/div>","protected":false},"excerpt":{"rendered":"<p>Zdravlje starijih osoba i ljetne vru\u0107ine &#8211; preporuke za za\u0161titu zdravlja Klimatske promjene i pridru\u017eene promjene vremenskih prilika, uklju\u010duju\u0107i porast temperature zraka, globalnog su karaktera.
